What is Foundayo?

Foundayo is an FDA-approved once-daily weight loss pill. It works by helping you feel less hungry, feel full sooner, and stay satisfied longer after meals. It's used alongside a healthy diet and regular physical activity for long-term weight management. Foundayo is not an injection — it's a tablet you swallow once a day.

How does Foundayo work?

Foundayo works by activating GLP-1 receptors in your body. GLP-1 is a natural hormone that sends signals to the appetite center in your brain to reduce hunger. By activating these receptors, Foundayo helps you feel less hungry and feel fuller after eating. It also slows down how quickly food leaves your stomach, which contributes to that sense of fullness. In addition, it helps improve blood sugar control by stimulating insulin release from your pancreas.

How is Foundayo different from other GLP-1 medications?

Unlike popular injections like Zepbound, Wegovy, Mounjaro, or Ozempic, Foundayo is a daily pill — making it a great option for patients who prefer not to use needles. But unlike oral Wegovy (oral semaglutide), which must be taken on an empty stomach with only a small sip of water, Foundayo can be taken at any time of day, with or without food. This makes it much easier to fit into your daily routine.

What are the benefits of Foundayo?

What side effects should I look out for?

The most common side effects are gastrointestinal in nature, including nausea, constipation, diarrhea, vomiting, and dyspepsia (indigestion). When these gastrointestinal side effects happen, they are typically mild to moderate in nature and generally subside over time, especially after your body adjusts to each new dose. Nausea is actually considered an “on target” side effect of Foundayo in that the nausea helps you to reduce your caloric intake and lose weight.
Foundayo may cause other less common, but more serious, side effects. These include:
  • Gallstones and pancreatitis (inflammation of the pancreas), especially when weight loss is rapid
  • Low blood sugar, particularly if you are also taking insulin or a sulfonylurea
  • Allergic reactions
  • Kidney problems, especially if you become very dehydrated
  • Changes in vision (in patients with type 2 diabetes and existing eye disease)
  • Thyroid tumors or cancer (this has been noted with other GLP-1 medications in rodents; orforglipron was not active in rodent thyroid cells, but it is listed as a precaution)
  • Hair loss (reported in about 4–5% of patients in clinical trials)

Who should avoid Foundayo?

People with any of the following conditions should not take Foundayo:
  • A personal or family history of medullary thyroid carcinoma
  • A personal history of multiple endocrine neoplasia syndrome type 2 (MEN 2)
  • An allergic reaction to orforglipron or any of its ingredients
  • Pregnant women (Foundayo may cause harm to a developing baby)
  • Severe liver disease (Child-Pugh Class C)

How do I take Foundayo?

Foundayo is taken as a once-daily tablet by mouth. You can take it with or without food. Swallow the tablet whole — do not break, crush, or chew it. Do not take more than one tablet per day. It does not matter whether you take it in the morning or at night, but try to take it at roughly the same time each day.

What if I miss a dose?

If you miss a dose, take it as soon as you remember. Do not double up on the next dose. If you miss 7 or more consecutive days, contact your Shapely provider — you may need to restart at a lower dose and re-titrate to reduce the risk of gastrointestinal side effects.

What dose of Foundayo should I take?

Most patients will start at the lowest dose (0.8 mg daily) for at least 30 days, and then increase gradually until we find the dose that works best for you. The maximum dose is 17.2 mg once daily. Make sure to attend your regular meetings with your Shapely provider so that we can find the dose that works best for you.

Important: Your provider may adjust this schedule based on how you are tolerating the medication. The goal is to find the dose that gives you the best results with manageable side effects.

What should I expect while taking Foundayo?

Because Foundayo slows down gastric emptying and signals your brain to be less hungry, you may feel fuller and naturally want to consume fewer calories. Many patients describe Foundayo as having an effect of reducing “food noise” or constant chatter and thoughts about food. You may also find a decreased desire to consume alcohol.
At the starting dose of 0.8 mg daily, there may be little to no weight loss. That dose is really intended to see how your body reacts to the medicine. As you increase the doses under the guidance of your Shapely team, you should expect to gradually lose weight. A healthy amount of weight loss is about 0.5–1% of your total body weight per week (usually 1–2 lbs for a 200 lb person), though weight loss may be greater when starting on treatment.

What foods should I avoid while on Foundayo?

While you don’t need to make any strict dietary changes while taking Foundayo, most people will find that they naturally want to consume fewer calories. In addition, improving the quality of your diet will only help to accelerate and sustain your weight loss.
Occasionally, Foundayo can increase symptoms of heartburn. If you have a history of heartburn, it’s recommended to avoid high-fat foods — like fried or greasy foods, as well as spicy or acidic foods.
Sometimes, patients will experience constipation while on Foundayo. As a result, it’s recommended to have a diet rich in fiber, as well as to consume plentiful water, as both can help minimize constipation.

Am I required to exercise while on Foundayo?

Exercise is not strictly required while on Foundayo, but it is strongly recommended! Many people who lose weight on Foundayo may also lose muscle mass. Doing exercise regularly will help ensure that you retain muscle mass and stay healthy.

Can I take oral birth control while being on Foundayo?

Because Foundayo slows down digestion, it may reduce the effectiveness of birth control pills. The decrease in effectiveness of birth control pills is strongest whenever you start a new Foundayo dose and improves over time. As a result, for women on oral birth control, it’s recommended to switch to a non-oral contraceptive method or add a barrier method of contraception for 30 days after initiation and for 30 days after each dose escalation.

What medications may interact with Foundayo?

Foundayo can interact with certain other medications. Make sure to tell your Shapely provider about all medications you are taking, including over-the-counter drugs and supplements. A few important interactions to be aware of:
  • Certain antibiotics (like clarithromycin) and antifungal medications may increase how much Foundayo is in your system, which could worsen side effects
  • Certain seizure medications (like carbamazepine) may reduce how well Foundayo works
  • If you take simvastatin for cholesterol, your dose should not exceed 20 mg daily while on Foundayo
  • If you take insulin or a sulfonylurea for diabetes, your doses may need to be lowered to avoid low blood sugar

Do I need to tell my doctor about Foundayo before surgery?

Yes! Because Foundayo slows down how quickly your stomach empties, there is a small risk of food remaining in your stomach even after fasting. Make sure to tell your anesthesiologist or surgeon that you are taking Foundayo before any planned surgery or procedure.
