For many Medicare beneficiaries, GLP-1 medications for weight loss have been out of reach — not because the medications don’t work, but because Medicare has historically not covered them for weight management. That’s changing. Beginning July 1, 2026, a new program called the Medicare GLP-1 Bridge will provide eligible Medicare Part D beneficiaries with access to GLP-1 medications approved for weight reduction at a $50 monthly copay (1).

In this post, we’ll walk through what the GLP-1 Bridge is, who qualifies, which medications are included, and what you should know about costs and next steps.

What Is the Medicare GLP-1 Bridge?

The Medicare GLP-1 Bridge is a short-term demonstration program run by the Centers for Medicare & Medicaid Services (CMS). It provides eligible Medicare Part D beneficiaries with access to certain GLP-1 receptor agonist medications that are FDA-approved for weight reduction — at a fixed copay of $50 per month (1).

The program runs from July 1, 2026 through December 31, 2027. It is designed to serve as a bridge to a longer-term initiative called the BALANCE (Better Approaches to Lifestyle and Nutrition for Comprehensive hEalth) Model, which is expected to launch for Medicare Part D in January 2027 and continue through 2031 (2).

It’s worth noting that this program specifically covers GLP-1 medications when prescribed for weight management. If you are currently prescribed a GLP-1 medication for another condition — such as Type 2 diabetes or cardiovascular risk reduction — your coverage will continue through your regular Part D plan.

Which Medications Are Covered?

Medications covered under the Medicare GLP-1 Bridge — Wegovy, Zepbound, and Foundayo at a flat $50/month copay

Under the GLP-1 Bridge, the following medications are eligible when used for weight reduction (1):

  • Wegovy (semaglutide) — injection and tablet formulations
  • Zepbound (tirzepatide) — KwikPen formulation
  • Foundayo (orforglipron)

Starting in January 2027 under the BALANCE Model, the list of eligible medications is expected to expand to include all formulations of Mounjaro (tirzepatide), Ozempic (semaglutide), and Rybelsus (semaglutide) (2).

What Will It Cost?

Eligible patients pay a flat $50 per month for their GLP-1 medication through the Bridge program. Both Novo Nordisk (maker of Wegovy) and Eli Lilly (maker of Zepbound) have agreed to participate at this price point (3).

There are a couple of important details to be aware of regarding how this interacts with your existing Part D benefits:

  • The $50 copay does not count toward your Part D deductible.
  • The $50 copay does not count toward the $2,100 annual out-of-pocket cap on prescription drug costs in 2026.

This means the Bridge operates as a separate, fixed cost alongside your regular Part D benefit — not within it (3).

Who Qualifies for the GLP-1 Bridge?

Do I qualify for the Medicare GLP-1 Bridge? Eligibility criteria by BMI 35+, 30+, and 27+

To be eligible, you must be enrolled in a Medicare Part D plan — either a standalone prescription drug plan (PDP) or a Medicare Advantage plan that includes prescription drug coverage. In addition, you must be 18 or older and meet one of the following clinical criteria at the time GLP-1 therapy is initiated (1):

  1. BMI of 35 or higher — no additional health condition required.
  2. BMI of 30 or higher with a diagnosis of at least one of the following: heart failure with preserved ejection fraction, uncontrolled hypertension, or chronic kidney disease (stage 3a or above).
  3. BMI of 27 or higher with a diagnosis of at least one of the following: pre-diabetes (as defined by American Diabetes Association guidelines), prior myocardial infarction (heart attack), prior stroke, or symptomatic peripheral artery disease.

Importantly, you do not need a diabetes diagnosis to qualify. The program is specifically designed for individuals who can benefit from weight reduction.

How Does the Prior Authorization Process Work?

Coverage through the GLP-1 Bridge requires a prior authorization, meaning your prescribing provider must confirm that you meet the eligibility criteria before the medication is covered. This is standard practice for many medications and helps ensure the program is used appropriately.

What Comes After the Bridge?

The GLP-1 Bridge is designed as a temporary program. Beginning in January 2027, the BALANCE Model is expected to take over and expand coverage. Under BALANCE, CMS will negotiate directly with GLP-1 manufacturers on behalf of Part D plans, with the goal of providing broader access to these medications at affordable prices through 2031 (2).
